Snellen Scale in 200, 100, 70, 50, 40, 30, 20, 15, 10 feet increments.

Published by National Society for the Prevention of Blindness, Inc. 1790 Broadway New York 19, N.Y. with a “40” in the lower left-hand corner. 9 x 22 ½ inches (width x length).

Paper has a black metal hanger bar and a black metal anchor bar at top and bottom, with a fat round flat silver metal hoop at top from which to hang the eye chart poster.

Currently protected and encased in a simple poster frame, with overall frame size being approximately 12 x 36 inches (width x height).
